Board Evaluation

Individual board members are asked to complete the following assessment of the Board as a collective.

Responses will be confidentially collected electronically. Results will be compiled to ensure anonymity and will be shared with the entire Board. All responses will be handled with the utmost confidentiality to encourage honest feedback.
